: We would like to get to know your child and family a little better to understand how we can help come alongside your efforts to raise them in the faith. We would appreciate you letting us know about your child, so we are better prepared to include them and make them feel welcome as part of our Faith Formation program. Please be aware, these forms will be held in confidence and only shared with the catechist assisting with your child’s particular grade.
